CONSENT TO APPEARANCE OF MINOR CHILD ON THINKTECH

In a talk show guest is a minor, we will need consent from a parent or guardian for him or her to appear in that shows. Here is a form of such consent:

Dear ThinkTech Hawaii:

I represent that I am the parent or guardian of the minor child described below. I am authorized to consent to my child's appearance and participation in a talk show at ThinkTech Hawaii and I hereby consent to the appearance and participation my child on that show.

I understand and agree that the video in which my child appears will be streamed live and posted on the Internet and on community and cable television and that ThinkTech may play, publish, broadcast or otherwise distribute or disseminate the video in any way it sees fit and will be under no obligation to edit or remove that video from the Internet now or at any time in the future.

I also understand and agree that that video and all rights in and to that video will belong and are hereby assigned to ThinkTech Hawaii and that neither I nor any other parent or guardian of the child described below, or the child himself or herself, will have any rights in or to that video.
